Want yo be an intern at Elevate Chiropractic and shadow Dr. Sayegh?

We are currently implementing a 6 week internship program where 1 lucky individual will be able to get hands on experience and shadow Dr. Sayegh and his trained staff. If you are interested, please fill out the information below letting letting us know why we should select you!

Qualifications:

1. ​Must be 18 years or older.
2. ​Must be attending a college or university with a passion of pursuing a carrier in the fields of Chiropractic, Kinesiology, Sports Medicine, Rehabilitation or Physical Therapy. 
3. Must be in good standing with a College or University. 
4. Must have a cumulative GPA of 2.5 or higher.

One person will be selected each quarter of the year. If you are interested and qualify, please fill out the information below and a staff member will reach out to you as soon as possible. Must be a local resident with proper transportation. 

What you need:

1. Complete resume.
2. A 30 second video on why you should be chosen (Video must not exceed 30 seconds and reflect your personality, ambitions, passion and reason for wanting to be select). 
3. Unofficial university or college transcripts.

The Process:

1. Phase 1: Submit application and required documents
2. Phase 2: Wait for a call back to schedule an Interview
3. Phase 3: Selection Process
4. Phase 4: You will be notified by phone, text or email if you are selected.
